How does Ketamine-Assisted Psychotherapy work?

Ketamine has long been used for anesthesia and pain relief - but about 20 years ago, scientists began to explore why depression improved in patients given ketamine in the OR. In 2019, one form of ketamine (esketamine nasal spray) became FDA approved for treatment-refractory depression. While there has been an emergence of studies done on the mental health benefits of Ketamine-Assisted Psychotherapy (KAP), the use of sublingual, troche, IV, and IM ketamine remains "off-label" for mental health indications. Nonetheless, mental health providers are finding that using ketamine to assist in psychotherapy sessions may help produce rapid change compared to traditional talk therapy. This may be due to the medication's antidepressant and anxiety-reducing effects which produce an altered state of consciousness. These therapeutic journeys use sub-anesthetic doses of ketamine to induce profound relaxation and tranquility, to allow deeper exploration. In contrast to most medications, which tend to elicit a masking response, ketamine serves as a gateway to consciousness, at times enabling individuals to discover the underlying causes of their symptoms, reshape their personal narratives, and even make therapeutic breakthroughs.

After a two-step screening process with our team, eligible KAP candidates move on to a focused psychiatric intake session with a partnering prescriber, as well as a psychological intake session with an MD. Next, a preparation session allows the client to discuss their treatment schedule before it begins, and prepare for their experience. This is followed by  6 to 8 dosing sessions of sub-anesthetic doses of ketamine. These sessions usually span across 2-8 weeks

During dosing sessions, patients take sublingual tablets or lozenges, which begin to take effect in 5-10 minutes when placed under the tongue. These sessions occur in person to allow us to monitor response, vital signs, and provide real time support. Virtual sessions may be offered on a case-by-case basis.

Dissociative effects typically last 60-90 minutes. Many people return to baseline 1-3 hours after treatment. We encourage patients to rest for the remainder of the day following treatment.

 

Patients are monitored by a licensed Medical Doctor and chaperoned by our dedicated staff. Each dosing session is followed by an integration session with an MD. After 2-8 weeks of KAP, a final integration session concludes the program. Additional integration sessions are available as needed.

An MD conducts each of the 20 sessions, and administrative support is available throughout the sessions as well.

Out-of-Network Reimbursement

Our clinic is out of network, meaning it is not contracted with any insurance companies. This allows us to keep our costs low, without the need for dedicated billing staff.  If you will be requesting reimbursement through out-of-network mental/behavioral health benefits, the common CPT codes used are:

Psychological Assessment

We evaluate your history, diagnoses, and prior approaches to ensure that ketamine-assisted psychotherapy is a safe and appropriate intervention for you. You would complete our screening process where we collect a comprehensive history.  You might also meet with an external ketamine prescriber, if you do not already have access to the medication.

We will work together to prepare your mind and body for the unique experience of ketamine-assisted psychotherapy. Before treatment, we will provide guidance to establish a positive mindset and prepare you for the dosing sessions and the integration process. We will support you in setting an intention for your journey, and answer any remaining questions.

Dosing Sessions

The customary process is to conduct 6-8 dosing sessions with the medicine within a span of 2-8 weeks. These sessions take place in our private office. *We prefer the dosing sessions to occur in person in our offices.* Your sessions will be accompanied by music curated to your life experience and intentions, and your vital signs will be monitored to uphold safety standards.

While each dosing session is followed by an integration session, a final integration will conclude the program, to assist in extracting insights and incorporating them into one's outlook. Our team will support you in making lifestyle changes and adopting practices tailored to your experience, empowering you to bring positive change to your daily life.

Follow Up

Our team is available for continued support after the program's sessions are complete. During the preparation process, we can discuss continued care and opportunities to follow up long-term, including more integration sessions between dosings, additional individualized integration sessions, or continued integration in a group format.

You may have access to psychedelic medicine in another context (i.e. an international retreat), and we can provide preparation and integration services. While we do not condone, source, or encourage the use of any illegal substance, our physicians work from a harm reduction model to reduce the risks from use in non-clinical settings. ​
 

The populations we specialize in, and may have an active integration group to join, are:

A co-ed group of individuals identifying as Black, Indigenous, and Persons of Color.

South Asian

A co-ed group for those from the subcontinent diaspora (including India, Pakistan, Sri Lanka, Nepal).​

Women (ages 25-45)

A group led by women, for women, interested in building community with like-minded women.​

FAQ: Will I be able to go back to work after a KAP session?

We recommend patients to take the rest of the day off. Even though the effects may wear off within 90-120 minutes of administration, dissociative and emotional effects can remain. Our KAP sessions are scheduled for the afternoon or evening for this reason.
